The article looks at the NSW model of Transition Education within the Department of School Education which has meant the introduction of an individual transition planning process (ITPs) for students with disabilities from 12 years of age onwards. The plans used in the 4 original pilot areas during 1989 and 1990 were examined and this article looks at the results which highlight issues that need to be addressed before individual transition plans can be successful, e.g. the inclusion of the students themselves; the need for on-going staff training and support.
