This article shows how generic primary care services can effectively co-ordinate their services to accommodate people with disabilities who have very high support needs. It gives a detailed account of a woman who has moved into supported accommodation after spending 13 years in a locked institutional ward. In order to optimise her physical and mental health, a local GP was sought and a complete assessment of all her health needs made. Thoughtful co-ordination of a professional team including anaesthetist, gynaecologist, dentist and opthamologist, together with careful and detailed planning by support workers has resulted in "J" being a much happier and healthier person.
